Terms of Sale:

Construction Force Services, Inc.'s terms of sale are payment due on reciept of invoice. All invoices not paid in a timely manner may cause interruption of credit extension unless arrangements are made with the credit department. Construction Force Services, Inc. will automatically assess a 1.5% finance charge per month on all past due invoices. Applicant agrees to pay any collection costs incurred to collect the account balance, including reasonable attorney's fees.

Applicant's Statement:

Applicant authorizes Construction Force Services, Inc. to verify or check any of the information given, check credit references, and obtain one or more credit reports in connection with this credit application or in connection with any periodic reviews of credit which may be extended to applicant. Applicant gives all of Applicant's creditors, banking relationships permission to give Construction Force Services, Inc any information it needs to make a credit granting decision. Applicant gives Construction Force Services, Inc. permission to report to credit reporting agencies and others the results of such investigations and Construction Force Services, Inc.'s experience with Applicant's account. Applicant agrees that exclusive jurisdiction and venue shall be in the state or federal courts in Queens County, NY.
