I hereby authorize construction force services (C.F.S.) to investigate the credit references listed herein as well as any public credit reporting services available, which I certify is true and correct, in order to establish a line of credit. C.F.S’ term s of sale are due upon receipt of invoice. All invoices not paid in a timely manner may cause interruption of credit extension. I authorize C.F.S. to charge all overdue invoices to my credit card. C.F.S. will automatically assess a 1.5% finance charge per month on all past due invoices. Applicant agrees to pay any collection costs incurred to collect the account balance, including reasonable attorney fees.
