I hereby authorize construction force services (C.F.S.) to charge my credit card the total balance due and I understand that I will assess a 1.5% finance charge . I certify that I am an authorized user of this credit card and will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form. All invoices must be paid in a timely manner. Applicant agrees to pay any collection costs incurred to collect the account balance, including reasonable attorney fees.
